Unfortunately, we are unable to travel for provide services.

Do you offer telemed/zoom/skype?
We do offer these services in some cases only for feedback. We want to meet you in person and get to know you better!

Do family members need to stay for patients’ appointments?
We do ask that a family member or close friend are present for interview and feedback. During testing we can only allow patient and examiner to work one-on-one. If a patient needs help with toileting, we need a caretaker/caregiver present for all appointments.

What time should I arrive to my appointments?
Please try to arrive 10-15 minutes early prior to diagnostic interview to ensure we get you properly checked in with your new patient paperwork. Have your paperwork filled out and bring your insurance card(s) and a photo ID. If you do not have your insurance card(s) and a photo ID we will need to reschedule your appointment. If you do not arrive on time we may need to reschedule your interview. This will delay your evaluation for several months in many cases.

What should I bring to my appointments?
For your diagnostic interview bring completed new patient paperwork, a list of current medication, a list of providers you currently see, your insurance card(s), a photo ID and any legal documents such as durable power of attorney (DPOA). For testing bring reading glasses and hearing aides. We provide coffee, tea and water. You are welcome to bring your own beverage and snack.

What is your cancellation/no show policy?
We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting much needed treatment. If an appointment is not cancelled at least 24 business hours in advance you will be billed, this will not be covered by your insurance company.
